HKPR Four County Dementia Network Coordinating Group
Purpose:
To serve as a vehicle to facilitate people and resources coming together locally, in the district geographic area of Haliburton, Northumberland, Peterborough City and County and the City of Kawartha Lakes and in cooperation with others in Central East and provincial partners. To improve the system of care: including service delivery, education and research for persons with dementia, their families and caregivers.
To build on existing relationships/linkages and local opportunities:
To provide framework for communication of activities both to and from CELHIN and Provincial Dementia Networks.
Objectives
a) To organize the group, to coordinate, develop forums for Dementia
Networking opportunities in the four county districts, regionally and provincially.
b) To communicate with stakeholder networks.
c) To profile dementia related issues in the four counties
The Coordinating Group tries to include representatives from among the following stakeholder groups but is not restricted to:
· Public Education Coordinators
· Psychogeriatric Resource Consultants
· Alzheimer Societies
(of Peterborough, Haliburton, & Northumberland and City of Kawartha Lakes)
· Psychiatric Assessment Services for the Elderly –
· Central East Community Access Centre
· Long Term Care Home – Charitable, Homes for the Aged and For Profit sectors
· Community Counseling & Resource Centre
· VON Respite Services
· Tri County Support Services
· Developmental Sector
· Veterans Affairs
· Family Health Teams
· Community Health Centres
· Community Support Services
About the HKPR Dementia Network:
What prompted the establishment of a Dementia Network in the Four County area?In 2002, the Ontario Provincial Alzheimer Strategy #9A began an implementation of regional dialogues on creation of Dementia Networks.How did the Coordinating Group get created?Service providers working with persons with dementia and their families met to discuss the development of a network that would represent providers working in the four counties of Haliburton, City of Kawartha Lakes, Peterborough and Northumberland.
It was decided that as many service provider groups already meet throughout the four counties and this group could build on their existing activities and networking structures. The local groups were invited to join as a network and thus the creation of a HKPR Dementia Network Coordinating Group.
